We welcome new patients who live within our practice areas.  As it often takes some time for records to be forwarded from your former practice all newly registered patients will be asked to complete a health questionnaire and are offered a consultation with the doctor. 

Medical treatment is available from the date of registration.  Please contact reception for further information.

Health Questionnaire to be returned with GMS1

Patients registering at Dunster surgery may be eligible to use the on-site dispensary; see the 5 reasons for using the dispensary if you are invited to.

You can return your completed registration form and health questionnaire to somccg.reception-dunstersurgery@nhs.net or alternatively bring your forms into either Dunster or Porlock Reception desks, preferably with ID or proof of address.

Temporary Patients

If you are ill while away from home or if you are not registered with a doctor but need to see one you can receive emergency treatment from the local GP practice for 14 days. After 14 days you will need to register as a temporary or permanent patient.

You can be registered as a temporary patient for up to three months. After three months you will have to re-register as a temporary patient or permanently register with that practice.

To register as a temporary patient simply contact the practice and complete the form below.   You cannot register as a temporary patient at a practice in the town or area where you are already registered.
